by Health Center members and their dependents; it is not a contract
or binding agreement - the Health Center reserves the right to change
benefits as needed at any time.
PLEASE NOTE: The Pharmacy Department services listed here are based
on members having other medical insurance with prescription drug
coverage. The Health Center offers the prescription services listed
below ONLY after your primary prescription insurance has paid its
portion.

(1) Must be preauthorized by the Health Center
and given by a participating provider.
(2) Treatment services require Medicare Part B
coverage - call the Health Center if you have questions
(3) Covers standard frames and lenses. Special
frames and lenses also available at the difference in cost compared
to the allowance for standard frames and lenses.
(4) Costs effective 1/1/2003 for a 30 day supply,
except for "vanity drugs". Vanity drugs, such as those
for toenail fungus, hair growth, and weight loss are covered at
the wholesale cost. Please note: the amount of the co-pay may
change because of drug costs to the Hillman pharmacy and because
of the level of insurance reimbursment. The terms Tier II
/ Preferred Brand and Tier III / Preferred Brand drugs are designations
made by the insurance companies, such as Blue Cross/Blue Shield
and Preferred Care. The fastest and easiest way for you to determine
how your drugs are classified is to call your insurance carrier
or doctor - NOT the Hillman pharmacy.
