(3) Covers standard frames and lenses. Special frames and lenses also available at the difference in cost compared to the allowance for standard frames and lenses.

(4) Costs effective 1/1/2003 for a 30 day supply, except for "vanity drugs". Vanity drugs, such as those for toenail fungus, hair growth, and weight loss are covered at the wholesale cost. Please note: the amount of the co-pay may change because of drug costs to the Hillman pharmacy and because of the level of insurance reimbursment. The terms Tier II / Preferred Brand and Tier III / Preferred Brand drugs are designations made by the insurance companies, such as Blue Cross/Blue Shield and Preferred Care. The fastest and easiest way for you to determine how your drugs are classified is to call your insurance carrier or doctor - NOT the Hillman pharmacy.

(5) Call (585) 473-2000 at the Hillman Center for an appointment.
